What paperwork did you actually bring home from the hospital, and did anyone here ever need it?

A friend is flying out next month and asked me what to bring home from the hospital. I gave him my list off the top of my head and then realised I have no idea whether it is the right list, because in the end I never needed most of it.

What I came back with: a discharge summary, the operation note, a photograph of the medicines I was given, and the fit to fly letter. What I did not think to ask for was anything from the follow up appointment, which in hindsight is the document that says I was cleared and on what basis.

So for people further down the road than me: what did you bring back, and more usefully, what did somebody at home later ask you for that you did not have?

X rays. My dentist at home wanted the images, not a report about the images, and it took a fortnight of emails to get files that would actually open. Get them on a stick before you leave and check they open on your own laptop while you are still in the building.

Mine was the implant sticker. There was a small metal anchor used in my shoulder repair and when I mentioned it to a clinician here the first thing they asked was what it was and who made it. I did not know. I had a discharge letter that said the procedure but not the hardware.

Got it eventually because the hospital keeps good records and answered my email, but it should have been in my folder from day one. If anything goes inside you, come home with the manufacturer, the model and the serial number, and photograph the packaging label if they will let you.

The short answer to what you should carry home is: everything a clinician who has never met you would need in order to manage your recovery without guessing.

In practice that is the operation note (what was done, by whom, on what date, with what findings), the discharge summary, the anaesthetic record if they will provide it, a full list of medicines given and prescribed with doses, details of any implant or device including manufacturer and serial number, histology or pathology results if any tissue was removed, imaging as files rather than as a report, the wound care and suture or drain removal instructions with dates, and the written follow up or rehabilitation plan. Ask for it in English, take paper and digital copies, and store one copy somewhere that is not the phone you travel with.

The reason for the length of that list is simple: the team who operated cannot examine you once you have landed, so the notes are the only thing that travels with you. Anyone picking up your care at home is either reading a proper record or working from your description of an operation you were asleep for. Alan is right that hardware details are the item people most often lack and most often need years later. The full version is in aftercare when you get home. What any of it means for your particular recovery is a question for the clinicians looking after you, not for this thread.

That is a better list than mine by some distance. Sending it on to him with the note that he should ask at discharge rather than at the airport. The imaging as files point is the one I would never have thought of.
